sorry, indeed you were not inserting an image
so th ebug can be reproduced when the dialog is fullscreen, but not when it is not

I tried uploading a file with .docx extension on a low res screen and the "Ok" button is still greyed out.
Setting a higher screen resolution "solves" the problem.

As the button is visible, I don't think it is a low resolution issue...
should it be due the capitals in the extension? JPG instead of jpg?
