This is not only a Forms issue but maybe a CMS one.
htmleditor2docbook.xsl from Forms and CMS both try to match the root tag. If CMS wins, forms tag are not even watched, and thus forms are ignored.
Proposed solution : apply-templates on '*' on the root level, and then match a '*' with low priority doing nothing, so that plugins may catch their tags.
