Checkbox should be ticked for mail sending

      When applying workflow on forms, if you select entries and you want to apply an action (Refuse, Ask complements), a popup is opened for email sending (if email action set on workflow action).
      The Problem is that the checkbox "Email sending" which is at the top of the popup is not ticked and all field of the popup are available.
      If we try to tick it a first time, it doesn't work and all fields become disabled.
      If you try a second time, it works.
      It is just the initial state of the checkbox which is not correct.

          It occurs the second time this popup is diplayed in case of email sending set on several workflow actions.
          There is also a problem on popup title which is not updated correctly
          Use case :

          • An entrie is selected, the workflow action "Refuse" is selected => The popup is opened, the email sending checkbox is ticked, the popup title is correct
          • Another entrie is selected, the workflow action "Ask complement" is selected => The popup is opened, the email sending checkbox is not ticked and the popup title has not been updated (it is "Refuse" instead of "Ask complement")
