How to reproduce

• connect to CMS
• edit a content
• add a form
• add a checkox

Actual behavior

• I am unable to add more than one check box per item of the form

Expected behavior

• I'd like to add multiple options for one item of the form

Remarks

• I suggest to mix radio and checkbox button, the difference is only about the multiple value, isn't it?
