How to reproduce

• Go to the demo : https://demo.ametys.org/nightly-intranet-cms/intranet/index.html
• go to the form tool
• open the form ask a question
• Modify the identifier of the name field with a number, for example: 1_name
  
• Go to preview and fill out the form
